HARLEM COMMUNITY HOSPICE SWING BED SUITE

Harlem Community Hospice Swing Bed Suite provides acute care for our patients who are experiencing distressing symptoms that cannot be managed at home or in a nursing home. Inpatient care is appropriate when:

  • The patient's symptoms necessitate 24-hour care
  • Death is approaching and care in the home is not possible
  • Temporary respite care is required for hospice families experiencing extreme caregiver burden, stress or fatigue

The goal of the Hospice Suite is to relieve the acute symptoms, so that the patient may return to his or her home or nursing home.

Harlem Community Hospice & Hospital Swing Bed Suite is staffed around the clock with specially trained nurses, and patients receive daily or more frequent visits from our hospice physicians. To maintain continuity of care, the patient's interdisciplinary Hospice Team continues to visit, as well, including the social worker, chaplains and volunteers.

Our Hospice Suite is beautiful, with a homey atmosphere that creates a small community of patients, families, friends and staff. There are accommodations for a loved one to spend the night, and pets are welcome, too, as they are often “family.”
