HOSPICE CARE IN NURSING HOMES

Hospice brings the best in Long Term Care together with our expert end-of-life care. We specialize in teamwork and in helping the patient stay in the environment they know and trust. We partner with the resident, family, and Nursing Home in decision-making. Services include:

• Consultation and coordination with Nursing Home caregivers
• Specialist level medical management to alleviate pain and other symptoms, to achieve comfort, and to make adjustments as symptoms change
• Nursing visits for assessment and coordination
• Emotional and supportive counseling for the patient and his or her family
• Enhancement of the patient's physical, emotional and spiritual comfort
• Aides who may provide personal care services
• Equipment and medical supplies as needed
• Bereavement support for family, friends and nursing home staff
• Inpatient care as needed

When in medical crisis, patients may be transferred from your facility to an inpatient unit for acute care. When the crisis has passed, the patient may return to their residence.
