FOR MEDICAL PROFESSIONALS
Referrals – Referring a patient to hospice is simple. We accept all patients with a limited life expectancy (prognosis 6 months) who want our service, regardless of diagnosis. Patients may participate in developing their care plan, including choosing their care team, which frequently involves their referring physicians. At Continuum Hospice Care, we respect doctor-patient relationships that existed before hospice was necessary and are glad to collaborate with physicians that patients know and trust. Our Medical Directors and nurse practitioners are available to consult with you regarding hospice care for your patient and his or her family. There is no charge for this service. |
Reimbursement – Whether part of the Care Team chosen by the patient or consulting as needed, physicians are reimbursed for the care given to hospice patients.
- The Physician of Record is designated by the patient at admission
and subsequent consultations are reimbursable by billing Part B directly.
- Physicians consulting on the terminal illness must be included in
the Plan of Care and have a contract with Continuum Hospice Care. CONTINUUM

When Hospice is Appropriate – We encourage physicians to refer their patients to hospice as soon as a life-limiting (6 months) prognosis is made. Hospice is a holistic approach to the end-of-life that includes helping the patient and his or her family cope and accept. Together, we can alleviate the burdens of pain, fear, confusion, and – all too often - inaction and turn this phase of life into an enriching time of peace and closure.
