HOSPICE CARE FOR SPECIAL NEEDS

At Continuum Hospice Care, we are committed to the principle that every person who needs and wants hospice care should have it. We create and provide programs for as many groups with special needs as possible. Our special programs include:

• Deaf and Deafblind Services – This culturally sensitive and linguistically appropriate hospice program provides communication access to patients, their families, doctors, nurses, social workers, special therapists and chaplains by way of qualified interpreters. Additional support stems from trained deaf volunteers as well as trained volunteers who can hear and sign. Continuum Hospice Care can assist deaf patients with deaf or hearing family members, and hearing patients with deaf family members.
  
  
• Hospice for People with HIV/AIDS – The skilled team of hospice healthcare professionals provide compassionate care to people with HIV/AIDS with specialized treatment and support for the distressing symptoms of this progressive disease. The patient's family of choice is welcome and encouraged to participate in the care of their loved one, and receives support through the course of illness or loss of a loved one.
  
  
• Hospice for People with Development Disabilities – We structure the Plan of Care to allow people with development disabilities to remain surrounded by loved ones, friends and familiar caregivers, whether in personal residences, in group homes or in specialized facilities. Specialized anticipatory grief and bereavement support is provided for the family of origin as well as the “extended family” of other residents with developmental disabilities and staff.
  
  
• Hospice for ALS Patients – Continuum Hospice Care collaborates with the ALS Center at Beth Israel Medical Center and the Eleanor and Lou Gehrig MDA/ALS Center in order to proved the most effective hospice care possible for ALS patients. Working with ALS specialists, our staff helps to minimize the day-to-day challenges involved with mobility and communication while introducing hospice treatments for discomfort related to the end of life.
  
  
• Alzheimer's and Dementia Patients – Our specially trained staff is ready to work with the family, medical professionals and caregivers of a person with advanced Alzheimer's disease or other types of dementia. We bring our specialized knowledge and skills related to end-of-life care to help the family make the wisest choices for keeping their loved one as comfortable as possible through the final stages.
